Please carefully read and understand the following terms and conditions before proceeding with any hair services at Dye Pretty Beauty Lab. By availing our services, you agree to the following:

1. Acknowledgment of Potential Risks:
I acknowledge that there are inherent risks associated with hair services, including but not limited to chemical treatments, hair coloring, haircuts, and styling. I understand that these procedures may carry potential risks of allergic reactions, skin irritations, or other adverse effects. I voluntarily assume any such risks.

2. Consultation and Disclosure of Information:
I confirm that I have provided accurate and complete information regarding my hair history, including any prior treatments, allergies, sensitivities, or medical conditions. I understand that failure to disclose such information may impact the outcome of the services provided, and I release Dye Pretty Beauty Lab and its staff from any liability related to any undisclosed information.

3. Consent for Services:
I consent to the recommended hair services, as discussed and agreed upon during the consultation with the stylist. I understand that the stylist will use their professional judgment to perform the requested services, and that results may vary based on individual hair type, condition, and other factors.

4. Limitation of Liability:
I agree that Dye Pretty Beauty Lab, its owners, stylists, and staff will not be held liable for any damages, injuries, or losses arising from the hair services received. This includes, but is not limited to, any allergic reactions, hair damage, dissatisfaction with the outcome, or any other adverse effects.

5. Release and Indemnity:
I hereby release Dye Pretty Beauty Lab, its owners, stylists, and staff from any and all claims, demands, actions, or liabilities arising from or related to the hair services provided. I agree to indemnify and hold harmless Dye Pretty Beauty Lab from any third-party claims resulting from my failure to disclose accurate information or any other negligent or intentional misconduct on my part.

Submission of this form indicates your agreement to the provided estimate, understanding that the final amount may be adjusted accordingly. I acknowledge that the stylist will address any inquiries and offer recommendations for a hair color maintenance program. However, neither the stylist nor Dye Pretty Beauty Lab can guarantee the exact outcome of my color unless i utilize the recommended products.

I have provided comprehensive information regarding any prior hair services received before the color correction service, including details about medications and vitamins. I comprehend that the estimate provided does not encompass subsequent visits necessitated by color fading and changes. Furthermore, i am aware that color correction is a gradual process that may require multiple visits to achieve the desired outcome.
